A. Requirements for the Accreditation/Renewal of Foreign Recruitment Agencies (FRA) / Direct Employers (DE) Hiring Professionals and Skilled Filipino Workers

  1. Job Order (JO) [Job Order PRA]. A JO is a document that indicates the manpower request of the employer addressed to the DMW-licensed Philippine Recruitment Agency (PRA), containing the classification of occupations, positions, number of workers required per position, salary and allowances per position.

    • For FRAs, the Job Order must be submitted in two parts:

    1. Job Order from FRA addressed to PRA [JO FRA]
    2. Manpower Request from DE addressed to FRA [JO DE]

  2. Standard Employment Contract (SEC) signed on all pages by the direct employer or his authorized representative, which incorporates the minimum provisions of employment. You may use any of the following:

    • DMW Standard Employment Contract [SEC]; or
    • A company prepared employment contract that includes the signed Addendum [Addendum]

  3. Recruitment Agreement [RA] or Special Power of Attorney (SPA) - This is an agreement between the FRA / DE, and PRA containing among others the responsibilities of the parties with respect to the orderly deployment, employment, and return of Filipino workers in a form to be prescribed by DMW.

    • The list of licensed PRAs can be found here: [licensed PRAs]
    • For FRAs, please provide the signed Client Service Agreement between FRA and DE.
    • For existing FRAs, no need to resubmit the Recruitment Agreement, unless due for renewal.

  4. Affidavit of Undertaking [Affidavit] (signed by FRA and DE) to monitor the employment of OFWs and to submit a report of significant incidents concerning OFWs.

  5. Company Profile. [Company Profile]

  6. Business License of the FRA and DE.

  7. Photocopy of valid Identification of FRA and DE signatories.

  8. Photocopy of Valid DMW license of PRA and photocopy of passport or valid ID of licensee.

  9. VERIFICATION FEE:

    • Hiring one (1) worker: AU$54.00
    • Hiring two (2) or more workers: AU$162.00
    • Payable to MWO-Philippine Embassy via Australian Post Money Order or Bank Cheque.
    • Please write the FRA / Company Name at the back of the Money Order.

  10. To provide express prepaid return envelope.

    • Fill out the return address details completely.

B. For Additional Job Order / Manpower Requests:

  1. Job Order/Manpower Request addressed to PRA.

    • For FRAs, The Job Order must be submitted in two parts:
    1. Job Order from FRA addressed to PRA.
    2. Manpower Request from DE addressed to FRA.

  2. Status of deployment of previously approved job order

  3. VERIFICATION FEE:

    • Hiring one (1) worker: AU$18.00
    • Hiring two (2) or more workers: AU$54.00
    • Payable to MWO-Philippine Embassy via Australian Post Money Order or Bank Cheque.
    • Please write the FRA / Company Name at the back of the Money Order.

  4. To provide express prepaid return envelope.

    • Fill out the return address details completely.
