For Filipino workers in Australia, Nauru, Tuvalu and New Caledonia who have started working with the company and are planning to have a vacation or those who are already vacationing in the Philippines, please submit the following requirements:

1. Original copy of Employment Contract (Permanent of Casual)
2. Original copy of Signed Addendum (Annex A) as hereto attached, to comply with the minimum requirement of the Department of Migrant Workers (DMW) in employing Filipino workers abroad.

An insurance policy may be submitted instead of the addendum, provided there is a provision of repatriation clause or medical evacuation to the Philippines, in case of death or severe illness of the worker.


3. Photocopy of Working visa
4. Photocopy of Worker’s Passport Bio Page
5. Photocopy of latest payslip
6. Paid self - addressed return envelope. (Please write your full name, complete address, contact number and email address)
7. Verification fee of AU$18 payable to MWO – Philippine Embassy can be made through AusPost Money Order or local Bank Checque.

• First-time users / changed employers will be required to set an appointment to the nearest DMW/POEA office in the Philippines or at the MWO in Canberra.
