Report of Death (ROD) and/or Shipment of Human Remains/Urn to the Philippines

The death of a Filipino citizen abroad should be reported to, and registered with, the Philippine Statistics Authority (PSA) through the Philippine Embassy/Consulate General having jurisdiction over the locality where the event took place. A. REQUIREMENTS FOR REPORT OF DEATH ONLY (for cases wherein human remains are not intended to be brought to the Philippines)

  • Duly accomplished and signed Report of Death form (only for deceased FILIPINO citizens) [Click here to download the form].
    • If filed in person, please accomplish 4 copies and sign before the Philippine Consul.
    • If sent by mail, four (4) original duly accomplished Report of Death Forms (either type written or legibly handwritten) all originally signed by the next of kin (NOK) and must be NOTARIZED by either the Philippine Consul, Notary Public or Justice of the Peace.

    Note: The form must be accomplished neatly. No erasures please as this will be the permanent record. The Embassy has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.

  • Original Death Certificate issued by the State Registry showing the personal circumstances of the deceased and to be apostilled by the Australian Department of Foreign Affairs and Trade (DFAT), plus three (3) certified true photocopies.
  • Original Certificate of Cremation (for cremated remains), plus three (3) certified true photocopies.
  • Proof of Filipino citizenship of the deceased: Four (4) certified true copies of the deceased’s Philippine passport; Australian Permanent Resident Card or Work Permit, or Dual Citizenship documents (Identification Certificate and Approval Order). (For human and cremated remains)
  • If reporting 1 year after the date when the Filipino national passed away, an Affidavit of Late Registration of Death is required. The Affidavit should be properly filled up, signed by the next of kin (NOK) and NOTARIZED by either the Philippine Consul, Notary Public or Justice of the Peace. [Click here to download the form].

All the original documents will be returned together with the official document to be issued by the Embassy.

Please note that in addition to the above-mentioned requirements, other documents might be required when deemed necessary by the Consular Officer of the Embassy.

Filing can be done through either of the following:

IN PERSON (please call the Registry Section at (02) 6273 2535 extension 233 to set-up appointment).

BY MAIL - send all the requirements and applicable additional documents along with the following:

  • AUD 45 payment for the Report of Death payable by a postal money order or bank cheque to the Philippine Embassy. If paying by credit card please provide the credit card name, number, expiry date and cvv number at the back of the card).
  • AUD 45 payment for Affidavit of Delayed Registration of Death, if applicable.
  • Pre-paid express return envelope (large size) with return address.
  • Please provide your cell phone number & email address to allow the Embassy to communicate with you regarding your application.

B. REQUIREMENTS FOR REPORT OF DEATH AND CONSULAR MORTUARY CERTIFICATE (human remains that has not been cremated will be shipped to the Philippines)

  • Duly accomplished and signed Request form for Consular Mortuary Certificate applications [Click here to download the form].
  • Duly accomplished and signed Report of Death form (only for deceased FILIPINO citizen) [Click here to download the form].
    • If filed in person, please provide 4 copies.
    • If sent by mail, 1 original copy notarized by notary public/lawyer plus 3 photocopies.

    Note: The form must be accomplished neatly. No erasures please as this will be the permanent record. The Embassy has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.

  • Original Death Certificate issued by the State Registry showing the personal circumstances of the deceased and to be apostilled by the Australian Department of Foreign Affairs and Trade (DFAT), plus three (3) certified true photocopies.
  • Original Mortuary/Funeral Director’s Certificate, stating that the body was properly embalmed/cremated in accordance with internationally-accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased, plus three (3) certified true photocopies.
  • Original Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease, plus three (3) certified true photocopies.
  • Original Transit Permit issued by the Local Area Health Service at the place where the transportation begins, plus three (3) certified true photocopies.
  • Proof of Identity of the Deceased (if non-Filipino): Four (4) certified true copies of the Australian passport.
  • Proof of Filipino citizenship of the deceased, if applicable: Four (4) certified true copies of the deceased’s Philippine passport; Australian Permanent Resident Card or Work Permit, or Dual Citizenship documents (Identification Certificate and Approval Order).
  • Information on the consignee/addressee/person, name of carrier, date and port of embarkation, port of destination and scheduled time of arrival.
  • Affidavit of Delayed Registration of Death is required if application is filed more than one (1) year after the occurrence. (For deceased Filipino citizen) [Click here to download the form].

Note: For COVID-19 related deaths

  1. The remains shall be cremated before shipment; or
  2. For remains that do not require cremation, the casket shall be permanently and hermetically sealed from the port of origin until burial. The burial must take place within twenty-four (24) hours after the issuance of clearance of admission into the country.

All the original documents will be returned together with the official document to be issued by the Embassy.

C. REQUIREMENTS FOR REPORT OF DEATH AND EMBASSY CERTIFICATION FOR CREMATED REMAINS THAT WILL BE BROUGHT TO THE PHILIPPINES INSIDE AN URN

  • Duly accomplished and signed request form for a Certificate to transport cremated remains [Click here to download the form].
  • Duly accomplished and signed Report of Death form (only for deceased FILIPINO citizen) [Click here to download the form].
    • If filed in person, please provide 4 copies.
    • If sent by mail, 1 original copy notarized by notary public/lawyer plus 3 photocopies.

  • Proof of Filipino citizenship of the deceased, if applicable: Four (4) certified true copies of the deceased’s Philippine passport; Australian Permanent Resident Card or Work Permit, or Dual Citizenship documents (Identification Certificate and Approval Order) (For human and cremated remains)
  • Affidavit of Delayed Registration of Death is required if application is filed more than one (1) year after the occurrence. (For deceased Filipino citizen) [Click here to download the form].

  • Four (4) certified true copies of the passport data page of the person who will bring the urn to the Philippines and flight details, if available (For cremated remains).

All the original documents will be returned together with the official document to be issued by the Embassy.
